Panthenol is one of the least controversial ingredients in cosmetics. It holds water, it smooths, it does not sting, it has an unremarkable safety file, and it turns up in everything from nappy cream to conditioner.

It gets a page because of a single claim attached to it, and because of where that claim ends up in the regulations.

What it does, which is real

Panthenol is provitamin B5. Applied to skin it converts to pantothenic acid, the active form. It is a humectant, meaning it draws and holds water, and it is a conditioning agent, meaning it makes a surface feel smoother.

On hair it coats the shaft and improves how it behaves: better slip, less static, less breakage during combing. That last effect is worth taking seriously for fragile hair, because mechanical breakage during styling is one of the main routes hair is lost, and it is the one that responds to conditioning rather than to pharmacology.

On skin it is used in barrier repair products and after procedures, and it is well tolerated, which is why it is a common choice for reactive skin alongside Glycerin.

What it does not do, according to the regulation

Panthenol is also sold as an ingredient that grows hair, and that is a different claim.

The US regulation covering over-the-counter products for external use as hair growers or for hair loss prevention names its subjects, and dexpanthenol, the close relative used in the same products, is on the list. Its conclusion applies to the whole category: “all labeling claims for OTC hair grower and hair loss prevention drug products for external use are either false, misleading, or unsupported by scientific data”.

That is a strong sentence and it is worth being precise about its scope. It covers hair growth claims for topical products. It says nothing about conditioning, and conditioning is what panthenol actually does well.

So the honest reading is: panthenol makes hair look and behave better, and there is no basis for expecting it to produce hair that was not there. Those are different products sold with the same photograph.

Thicker is not more

Much of the marketing here trades on an ambiguity. A conditioning agent that coats the shaft makes each hair very slightly thicker and makes the whole head look fuller, and that is a genuine, visible, immediate effect.

It is also entirely cosmetic and reverses when you stop. Nobody is lying when they say hair looks thicker. The inference the picture invites, that there is more hair, is the part the regulation is about.

Where it fits in a routine

Panthenol is water-soluble, so it rinses. It does not build up, which puts it on the easy side of the trade that Amodimethicone sits on, and it is a reasonable ingredient to look for in a leave-in where buildup is a concern.

It combines with almost anything, it is stable, and it has no meaningful interaction problems. There is very little to say against it, which is unusual enough on this site to be worth stating plainly.

The names

Panthenol, D-panthenol, DL-panthenol and provitamin B5 are the same thing on a cosmetics label. Dexpanthenol is the name used in pharmaceutical and drug contexts, including in the regulation quoted above.

Pantothenic acid is what it becomes, and a supplement listing pantothenic acid is the oral version of the same vitamin, which is a separate question from what a conditioner does on the outside.
