Sulfate-free shampoo has to clean with something. Usually it is this.

Cocamidopropyl betaine is an amphoteric surfactant, meaning it carries both a positive and a negative charge, which makes it milder than the anionic surfactants it partly replaces. It also thickens, it foams, and it reduces the harshness of whatever else is in the formula. It is made from coconut oil, which is where the coca in the name comes from and why it reads as natural.

It is on this site because of a mismatch between how it is positioned and what the contact dermatitis literature says about it.

The gentle one is on the patch test panel

Cocamidopropyl betaine is a recognised patch test allergen, which is why it appears in diagnostic series. The European Society of Contact Dermatitis guideline lists it among the chemicals for which irritant potential “is more predominant”, alongside benzoyl peroxide, propylene glycol and benzalkonium chloride, and notes this frequently results in “weak erythematous (doubtful) test reactions”.

So it does two things: it can sensitise, and it can irritate, and the two are hard to tell apart on a patch test.

The practical version of that: someone whose scalp reacts moves to a sulfate-free shampoo, the reaction continues, and they conclude that nothing works. The ingredient they moved to is the one with the entry in the patch test series.

That is not an argument for going back to sulfates. It is an argument that “gentle” is a marketing category rather than a hypoallergenic one, and that if a switch did not help, the new formula is worth reading rather than the old one worth blaming.

The subtlety about what actually sensitises

The evidence points at manufacturing residues rather than at the surfactant itself. Cocamidopropyl dimethylamine and amidoamine are intermediates left over from production, and they are widely considered the actual sensitisers in most reported cases.

That matters because purity varies between suppliers, and two products listing the identical INCI name can differ in the thing that causes the reaction. Nothing on a label distinguishes them.

The coconut question

It is derived from coconut oil, and people with a coconut allergy reasonably ask about it.

The honest answer is that heavy chemical processing generally destroys the proteins that provoke a food allergy, so a reaction to this ingredient is more likely contact allergy to the surfactant or its residues than an extension of a food allergy. That is the same reasoning that applies to highly refined oils under the allergen rules.

It is also not a promise, and coconut is not a declarable allergen in cosmetics in most markets, so nothing on the pack addresses it. If you have a significant coconut allergy this is a question for your allergist rather than for a label.

Where it sits in a formula

Almost always alongside something else. A shampoo built on Sodium lauryl sulfate often contains cocamidopropyl betaine to take the edge off it. A sulfate-free shampoo often has it as the main cleanser, supported by glucosides.

Reading the first three or four ingredients after water tells you which kind of formula you are holding, and that is more informative than the claim on the front. The wider argument about sulfates and what they are actually doing is on the curly and coily hair page.
