Pyrithione zinc is the active in most anti-dandruff shampoos, and it is the ingredient that best illustrates something people rarely notice about that aisle: an anti-dandruff shampoo is a drug, not a cosmetic.
It has a monograph, and a range
The US drug regulations list the actives permitted “for the control of dandruff, seborrheic dermatitis, or psoriasis”, each with a concentration range. Pyrithione zinc appears at “0.3 to 2 percent when formulated to be applied and then washed off after brief exposure”.
A stated range, a stated use pattern, and a stated form. That is a very different regulatory footing from a cosmetic, where no concentration is disclosed and no claim is pre-cleared. It is also why these products carry a drug facts panel while the conditioner next to them does not.
The range matters practically. A product at 2 percent and one at 0.3 percent are both compliant, and the pack usually states which, because a drug facts panel names the active and its strength. That makes this one of the few places in a bathroom where you can read the dose.
The two usage instructions nobody follows
Dermatology guidance for high curl pattern hair is unusually concrete here, and both points cut against ordinary habits.
Apply it to the scalp, not the hair. The guidance is that “the shampoo should be applied directly to the scalp rather than the hair shafts to minimize dryness”. The active is working on the scalp. Running it down the length of the hair delivers it where it is not needed and dries out the fibre, which is the complaint people then blame the product for.
It does not need to sit. “Patients should be informed that prolonged contact time is not needed for medicated shampoos to be effective.” The advice to leave it on for five minutes is folklore that costs you moisture for no additional benefit.
Between them these two points resolve most of the reason people abandon medicated shampoos: they are experienced as harsh, and a large part of that is delivery rather than the drug.
The same guidance notes that anti-dandruff shampoos containing zinc pyrithione, selenium sulfide or tar should be used “carefully to avoid hair shaft damage and dryness”, particularly for fragile hair. Careful, not avoided.
Why a scalp problem is a hair problem
The connection is underrated and it runs in the direction people do not expect.
Washing less to protect fragile hair is a common and well-intentioned strategy, and it raises the risk of seborrheic dermatitis, because the scalp environment that produces flaking is the one left undisturbed. So the practice adopted to save the hair creates the condition that requires the harsher product.

The alternatives in the same regulation
Coal tar is the other classic, permitted at 0.5 to 5 percent, with its own labelling requirements. Selenium sulfide and ketoconazole are the other common actives.
Salicylic acid appears in this category too, working differently: it lifts scale rather than acting on the organism, which is why it is sometimes combined with one of the others rather than used alone.
The name
Zinc pyrithione and pyrithione zinc are the same thing with the words swapped, and both appear on packs. The INCI name is Zinc Pyrithione; the drug regulation calls it pyrithione zinc. Nothing turns on the difference.
