Modified food starch
Two separate worries, both usually unfounded, attached to one of the most common ingredients in processed food.

Starch on its own is not very cooperative. It thickens, then thins again when heated too long, separates when frozen, and turns watery when acidic. Modified starch has been treated so it stops doing those things, which is why it is in sauces, soups, yoghurts, fillings and almost anything that has to survive a freezer or a long shelf life.
The regulation covering it in the US is titled, straightforwardly, “food starch-modified”.
“Modified” does not mean genetically modified
This is the most common misreading, and it is worth clearing up because it sends people’s attention to the wrong place.
The modification is physical or chemical treatment of the starch itself: heating, acid treatment, cross-linking with approved reagents. It is a process applied to an extracted starch, not a change to the DNA of a plant. A starch can be modified and not genetically modified, or from a GM crop and not modified in this sense. The two words are unrelated and the label is using the technical one.
The question that does matter: which grain
For anyone avoiding gluten, this is the real one.
Modified food starch can be made from corn, potato, tapioca or wheat. In the United States it is most often corn. If it is made from wheat, wheat is a major allergen and US allergen labelling requires the source to be declared, so the label will say so. In the EU, cereals containing gluten must be named and emphasised in the ingredient list.
So “modified food starch” with no grain named on a US label is not wheat. That is a real, enforceable inference, and it is the reason this page ends up in the reassuring category rather than the anxious one.
If a product also carries a gluten-free claim, the 20 ppm threshold in 21 CFR 101.91 applies to the finished food regardless of what the starch came from, which is a stronger assurance again.
Why it gets flagged anyway
Because it sounds industrial, and “modified” reads as a warning to an English speaker in a way that “starch” does not. That is a reaction to vocabulary rather than to evidence, and it is the same mechanism that makes Potassium sorbate look alarming and an E number look like a code for something being hidden.
The useful move is to ask what the word actually means in context, which is most of what reading a label comes down to.
